Prior to your scheduled appointment, please scroll below to our "New Patient Forms" tabs and then download and complete the appropriate patient registration form and bring it with you on the day of your appointment.
Please bring your Identification Card (ID), as well as the most current health insurance cards with you on the day of your appointment. If you have an insurance plan that requires a referral, please call your primary care provider (PCP) and have them initiate a referral to the provider you are seeing here at MEC Eye Specialists. The referral must be dated for the day of your appointment here.
If your insurance requires you to pay a co-payment, this will be due at the time of service. If your insurance does not cover the refractive portion of your eye exam, you will be responsible for that payment at the time of your exam.
Please be advised that a recommendation from your primary care physician (PCP) to see a MEC Eye Specialists provider does not guarantee payment by your insurance plan. If you have any questions regarding your health insurance coverage, please contact your health insurance plan prior to your visit.
If you have an appointment with our Ophthalmologist or any of Sub-specialist physicians please be advised that an initial appointment could take up to 2 hours.
